Home Health Services

Jackson Medical Center also offers Home Health Services. Several studies have shown that some patients recover faster and live longer when they receive health care in their own homes. Home health can be more cost-effective than other options such as continued hospitalization or nursing home placements.

Home Health is offered to patients who have Medicare and are homebound. Medicare reimburses Home Health 100% for their services, if the patient meets all admission criteria and the service is prescribed by a physician. Medicaid and most private insurance companies may also pay for home health services.

Home Care Services Available to You

Skilled Nursing

A Registered Nurse or Licensed Practical Nurse will visit the patient to perform skilled services such as:

Assessment & instruction to include vital signs, disease process, medications, dietary regimen and patient care management

Ulcer & Wound Care

Insertion of Catheters & Feeding Tubes

IV Therapy

Physician contacts as indicated by patient's condition

Chemotherapy Pain Management

Diabetic Education

Follow-up with Patient/Caregiver

Home Care Aide

Under the direction of a registered nurse, the home care aide provides personal care for the patient which includes:

Bathing

Exercise & Mobility

Skin, Hair & Nail Care

Physical Therapy

The physical therapist carries out the physician's orders for treatment to relieve pain and restore abilities by:

Designing individual exercise programs

Instructing in the use of wheelchairs, crutches, canes and other appliances

Gait Training

Mobile Ultrasound
